This type of plan is available on a voluntary basis to MassHealth members and covers the services reimbursable under Medicare and MassHealth. This program offers MassHealth Standard and Medicare Parts A and B members aged 21-64 quality care that combines health services with social support services.

A team of professionals, including a member’s primary care physician, nurses and a Long-Term Support Coordinator, works with the members and their families to develop their plan of care, which combines health services with community based services.
